Medical Record requests can take up to 1 week to process. We cannot e-mail them. It can only be mailed, faxed or dropped/picked up. We will need an Authorization for Release of Medical Record (ROI) form signed by parent/guardian before anything can be released. If you are transferring care to another facility, the new facility will give you its ROI form. We have our own ROI form here that you can fill out as well.
